Transitions

Fox Hollies School, transition into adulthood is carefully planned and personalised for every student.

From Year 9 onwards, we work closely with students, families and professionals to plan next steps based on each student’s:

  • strengths and interests

  • level of independence

  • communication and support needs

  • Preparing for Adulthood outcomes

We support students and families through:

  • annual EHCP reviews with a focus on next steps

  • visits and taster sessions to colleges and providers

  • multi-agency working with education, health and social care

  • regular communication with families

Students move on to a range of destinations, including:

  • further education and training

  • specialist colleges or supported learning

  • supported employment pathways

  • community or adult provision

We recognise that transition looks different for every student. Our aim is to ensure that each student moves on to a setting where they feel safe, confident and able to continue developing.

We support students to:

  • understand their needs and preferences

  • take part in decisions about their future

  • develop the confidence to move into adult life

Internships

For a small number of students that can aim to employment in adulthood. Funded by SENAR, requested in the EHCP review.


Education

For students continuing specialist education funded via SENAR and requested in the EHCP review.


Adult Social Care

For students that no longer require formal education and would benefit from daily meaningful activities in a long term day centre placement. The EHCP plan will cease and day centre provision will be assessed and funded with the adult transitions social care team.
